In order to submit your rebate information to AirZone, you will need to have the following forms signed and completed:
FOR GAS and ELECTRIC customers (1 form required):
Heat Pump Participation Agreement
FOR OIL and Propane Customers (2 forms required):
Heat Pump Participation Agreement
Once these forms are completed, you will be able to upload the signed files using the below form.
